Moreland OB-GYN Associates, S.C. is here to assist you with the completion of your disability of FMLA form. We have trained staff who work closely with the physicians to complete and submit your medical leave paperwork to either your employer or disability company.

Completion of disability or FMLA form takes approximately 7-10 days. Your paperwork can be submitted to our office by email, fax, mail, or you can drop it off at the front desk with the office staff.

We ask that you submit all of your medical leave forms. Please make sure that the information section has been completed and signed. Your signature is required in order to release information.
